 MONMOUTH COUNTY FAIR
4-H Animal Health Requirements


In order to maintain the health of all patrons, exhibitors and animals, the following forms are required for 4-H members exhibiting live animals at the Monmouth County Fair.

All documents must be received before July 5, 2024 and must be valid through July 28, 2024. FORMS MUST BE LEGIBLE. Double copies and photos of forms are not acceptable. Failure to meet these requirements will prevent the animal from entering the fairgrounds.
  
The following documents are required for all 4-H animals participating in the 2024 Monmouth County Fair. These requirements are based on the New Jersey Department of Agriculture’s 2024 Fair/Show Animal Health Recommendations, Regulations, and Forms; Monmouth County 4-H Animal Projects Registration, Health, and Event Information; veterinarian recommendations and public health guidelines.
  
Additional requirements for animals originating OUTSIDE OF NEW JERSEY (Imported) are noted in red.
  
Please note that Animal Health Certificates (AHCs) and Certificates of Veterinary Inspection (CVIs) ARE NOT interchangeable – AHCs will not be accepted for imported animals.
  
4-H members are responsible for the proper care and upkeep of their livestock and enclosures.

HORSES  
** These forms require a Veterinarian’s signature.

RABBITS & CAVIES  
SMALL ANIMALS / HERPETOLOGY
** These forms require a Veterinarian’s signature.
 
GOATS
** These forms require a Veterinarian’s signature.
  
SHEEP
** These forms require a Veterinarian’s signature.
  
SWINE

  • 4-H Animal Registration Form
  • 4-H Animal Lease/Borrow Agreement (if applicable)
  • NJ AHC, NJ Fair AHC or NJ CVI within 30 days ** (See sample)
  • State of New Jersey Drug Use Form
  • Official CVI from originating state** (See sample) The CVI must contain a statement from the issuing veterinarian that he/she personally inspected the pigs, and they are free of clinical signs of infectious contagious or communicable diseases or known exposure within 30 days of shipment.
  • Negative brucellosis test within 30 days of entry (if applicable, see NJ regs)**
  • Negative pseudorabies test within 30 days of entry (if applicable, see NJ regs)**

CATTLE
  • 4-H Animal Registration Form
  • 4-H Animal Lease/Borrow Agreement (if applicable)
  • NJ AHC, NJ Fair AHC or NJ CVI within 30 days ** (See sample)
  • State of New Jersey Drug Use Form
  • Tattoo, brand, microchip or color wallet photo
  • Official CVI from originating state ** (See sample)
  • Negative tuberculosis test within 60 days of entry (if applicable, see NJ regs) **
  • Negative brucellosis test within 30 days of entry (if applicable, see NJ regs) **
  • Negative anaplasmosis/bluetongue test within 30 days of entry (if applicable, see NJ regs) **
  • ISO 11784/895 or NUES Eartag, tattoo, or registration papers with name and number.
** These forms require a Veterinarian’s signature.
  
POULTRY
  • 4-H Animal Registration Form
  • 4-H Animal Lease/Borrow Agreement (if applicable)
  • 4-H Poultry, Rabbit or Cavy Health Form within 30 days, OR-
    • NJ AHC, NJ Fair AHC or NJ CVI within 30 days ** (See sample) OR-
    • VS Form 9-2 / VS Form 9-3
  • All Poultry (with the exception of waterfowl, pigeons, and doves) and hatching eggs:
    • Shall originate from U.S. Pullorum-Typhoid Clean or equivalent flocks; OR-
    • Shall have an individual negative Pullorum-Typhoid test performed within 90 days prior to the exhibit by meeting the testing requirements below:
      • Poultry (with the exception of those types listed below) shall be tested when more than 4 months of age. Poultry under 4 months shall originate from Pullorum-Typhoid negative flocks.
      • Turkeys shall be blood tested when more than 12 weeks of age. Turkeys under 12 weeks of age shall originate from Pullorum-Typhoid negative flocks. Turkeys shall only be tested by NJDA authorized personnel.
      • Game birds (includes quail, chukar, grouse, and pheasant) shall be tested when more than 4 months of age or upon reaching sexual maturity, whichever comes first. Game birds under 4 months shall originate from Pullorum-Typhoid negative flocks.
      • Ostrich, emu, rhea, and cassowaries shall be blood tested when more than 12 months of age. All ostrich, emus, rhea, and cassowaries less than 12 months shall originate from Pullorum-Typhoid negative flocks.
      • Waterfowl, pigeons, and doves are exempt from Pullorum-Typhoid testing.
    • Call NJ Division of Animal Health at 609-671-6400 for certified testers.
  • State of New Jersey Drug Use Form
  • Official CVI or VS Form 9-2 or 9-3 ** (See samples - Official CVI | VS Form 9-2 | VS Form 9-3)
  • All poultry (except doves and pigeons) shall originate and move directly from NPIP US Avian Influenza Clean program or NPIP US H5/H7 Avian Influenza Clean or Monitored program, or a test negative monitored/tested flock (over 7 days old)

CANINES
Dogs must be leashed unless performing and are not permitted to leave their event site.
  
Dogs must be removed from the Fairgrounds at the conclusion of their event.
