Please read all notes below prior to filling out the forms:

Pre-participation physical forms must be completed by all prospective varsity student-athletes (all new freshmen and transfers) and returning student-athletes.

The medical portion of the physical will need to be completed by the student-athlete's own physician. This can be done in conjunction with the physical that is required of all new/transfer student-athletes prior to being accepted to SCSU when they go to see their physician.

***New Student-Athlete Medical Process*** COMING SOON, AVAILABLE ON JUNE 15, 2021. 

Sickle Cell Trait testing is REQUIRED for ALL student-athletes prior to participation.

This is mandatory by the NCAA and an institutional policy for athletic participation at SCSU. All NEW/TRANSFER student-athletes must have the sickle cell trait test results indicated on the physical portion of the form with a copy of the results attached.

***Returning Student-Athlete Medical Process***

All returning student-athletes who have previously had the sickle cell trait test done and the results are on file with the sports medicine department DO NOT have to have another test done. Physicals must be done by your personal physician and paperwork returned no later than June 30th.

This form is not to be completed by those students who will be participating in club sports.

The Southern Connecticut State University Sports Medicine staff is responsible for providing medical coverage, recognition and treatment of injuries and rehabilitation to all Owls student-athletes. The athletic training staff provides athletic injury care to all 19 intercollegiate athletic teams during the course of the year.

The staff consists of one head athletic trainer, several assistant athletic trainers and numerous student assistants.
