HOW TO MAKE A VACCINE APPOINTMENT

You do not need insurance or immigration documents to receive one

ALL DISTRICT RESIDENTS Age 5 and Older May:

  • For vaccines in Cook County, sign up on this website or call 833-308-1988 8am-8pm M-F. Walk-ins are also welcome at the locations listed on this website.
  • Information about Chicago sites, including local pharmacies and community health centers, can be found HERE.
  • Find a vaccination site via vaccine.gov, the State website, or by calling the Illinois Department of Public Health hotline at 833-621-1284. The call center is open 7 days a week 6am to midnight and can take TTY calls.
  • Text your 5-digit ZIP code to GETVAX (438-829). You'll receive a text with three vaccination sites near you with vaccines available. If you would like this information in Spanish, text your ZIP code to VACUNA which is 822-862. You can also click on the text or go to vaccines.gov to find a specific vaccine.
  • Call 1-800-232-0233 to find vaccination sites near you.
  • Check with your healthcare clinic to see if it is scheduling vaccine appointments.

CHICAGO:

All City of Chicago-operated vaccination sites are accepting walk-in vaccine appointments. Although appointments are no longer required, they are still encouraged. Information on all city-operated sites can be found here.

All residents age 5 and older are eligible for a free in-home vaccination. Each household that gets an in-home vaccine will receive a $25 Visa gift card. Book an appointment by calling 312.746.4835 or visiting www.chicago.gov/athome. Both Pfizer and J&J vaccinations will be offered.

Visit the Chicago Zocdoc website to find nearby vaccination locations and to book an appointment. Embedded translation support is available for more than 100 languages, including Spanish. Residents can also call the CHICAGO COVID hotline at 312-746-4835 from 8am to 8pm seven days a week for help signing up for a vaccine appointment. A list of other options for getting vaccinated (such as reaching out to your healthcare provider) can be found at this website.

Chicago residents age 5 and older are now also eligible for vaccination in Indiana even if they reside in Illinois. Registration can be completed at this website or by calling 866-211-9966.

Erie Family Health has COVID-19 vaccine appointments available for anyone 5 or older. You do not need to be a patient to sign up for a vaccine. Check for walk-in opportunities or schedule an appointment here or call 312-666-3494 or 847.666.3494, Option 5.

SUBURBAN COOK COUNTY:

Homebound residents can fill out this form online to register for an in-home vaccination or call 833-308-1988 Monday through Friday 8am-8pm to register to receive a vaccine at home. Those who register by phone will receive a call back from Cook County to schedule a dose.

EVANSTON:

Complete the City's Vaccination Interest Form here (también disponible en español), and the City will notify you when a vaccine appointment is available. Please complete this form for each member of your household regardless of age. More information can be found at this website.

For home-bound residents 65 years or older: please complete this form so the City can coordinate an in-home vaccination. This form can be filled out on someone's behalf.

SKOKIE:

To receive a vaccine through a Skokie-run event, residents must sign up through pre-registration here. You can also call 847-779-7726. The Village is currently offering doses to all individuals 5 and older. Details on the registration process and upcoming vaccine clinics can be found here.

If you have already received your vaccination in a location other than a Skokie-run event, please fill out this form to let the Village know you can be removed from the vaccination pre-registration list.

OTHER UPDATES

If you receive either the Pfizer or Moderna vaccine you MUST receive both vaccines to receive maximum immunity. For both your health and the health of those around you, please do not skip your second vaccination.

A grassroots network of organizations and individuals called the COVID-19 Community Corps has been launched by the Department of Health and Human Services to help with vaccine education. Community Corps members include NASCAR, Major League Baseball, American Medical Association, the Chamber of Commerce, the Farm Bureau, NAACP, and The Salvation Army. Vaccine Safety Updates for Specific Diagnoses: Those with HIV are recommended to receive COVID vaccines. And the CDC and the American College of Obstetricians and Gynecologists are recommending that those who are pregnant get the COVID-19 vaccine.
