This form is intended for use by current UMKC students in a dual degree program.

To use this form, you must have already submitted a Declaration of Major (DOM) on your undergraduate program to enroll in one of these programs:

You should only complete this form if you have applied to graduate from your bachelor's degree and are ready to transition to your master's degree courses. This form will create the master's degree application needed to switch your student status from undergraduate to graduate.

If you are not in a formal dual degree program at UMKC, please use the general UMKC application to apply to your graduate program (Master's or PhD).
