2023 CONTINUING EDUCATION REQUEST FORM

PLEASE REVIEW REQUIREMENTS BEFORE REQUESTING APPROVAL

Please review the Pre-Approved Continuing Education (CE) Providers and the Approved Events list found on the calendar page before submitting your approval request.

If the hosting organization is not listed on the pre-approved provider list, please review the requirements below before requesting CE approval.

• Applications must be received at least 2 weeks prior to the event and processed in the order in which they were received. Please do not request approval for past events.
• These CE opportunities may take the form(s) of webinars, lectures, courses, seminars, workshops, or symposium and meet the criteria as stated in the CE Policies and Procedures.
• Non-instruction time such as lunch and breaks are not considered part of clock hours.
• In order to record the CE for credit you will be required to provide a verification of attendance for ALL approved events.
• This can be in the form of a certificate of completion or letter from the event host on letter head provide name, date, approved CE hours. It is the responsibility of the member to obtain a proof of attendance.
• Upon submission, applicants will receive an automated submission confirmation containing a copy of their application.
