By filling out the above information and returning it, I understand that my organization is requesting a review of our website(s) to identify any language that may be or is stigmatizing.   *
I understand that after a review by the WV Collegiate Strategic Prevention Framework Partnership for Success Student Leaders, findings and recommendations will then be turned into the Project Director, Heather McDonnell-Stalnaker and Lead Epidemiologist, Katelyn Fowler for a final review.  Once a final review is completed, findings and recommendations will be sent back to the contact person listed above.   *
I understand that two weeks or one month (whichever you chose above) after the final findings and recommendations have been sent to the organization, the website will be reviewed again.  If the organization decides to make corrections to their website based on the findings and recommendations, the WV C-SPF-PFS will designate their website a Stigma Free Website and provide a digital badge to be displayed on the website.   *
