Request for a Bus Pass for the next school year ONLY IF changing schools (example: Going from Elementary to Middle or Middle to High) or did not ride the bus this year and will ride next year.

BUS PASSES are only valid for 5 days from date printed. If student does not turn the bus pass into the driver before 5 days, you will have to request a new bus pass.

•ONE FORM PER STUDENT• This form is for students living within their new schools boundary and outside the 2 mile walk zone only. For Dual Custody, Specialized Transport (ESE, PK, TPP, OPS, Medical 504) or SIT/Foster Students please contact school to arrange for busing.
If you move over the summer, please update your address with the school first and then submit a new request. This form does not guarantee the student a bus pass.
This form is not for students going to Choice Schools, Charter Schools or ELO/EPO Programs outside their zoned school.

If your address is not changed in FOCUS, we can not assigned you a bus pass until it is done. If you are not enrolled into the correct school, we can not issue you a bus pass until you are enrolled correctly. If we can not find you in the student system because of incorrect information, we can not issue a bus pass.
