MPCS Student Absence Form
If your student(s) will be absent or planning to miss multiple days of school, please complete the following form. For illness, the school clinic will receive the information and communicate with administration. The information submitted in this form is only available to our Nurses and school Registrars. If you have privacy concerns, you can email our nurses directly at nurse@mtparanschool.com.

Note: All absences that are not excused as defined by the handbook are considered unexcused. Parents need to consider the effect their child's absence in any particular class will have on their performance, final grade, exemption from final, and possibility of no credit due to excessive absences.
