This report should be completed in the event there is a policy or behavioral related concern pertaining to an individual student or Recognized Student Organization (RSO) at Missouri University of Science and Technology.

Policy compliance may pertain to the Standard of Conduct, Recognized Student Organization Policy, Standard of Conduct for Student Organizations, Social Guidelines, Constitution and By-laws for Interfraternity Council and/or Panhellenic Council, (Inter)National policies and procedures, or any federal, state, or local law.
