Please use this Incident Report Form to submit:
- Formal Report of Student Misconduct
- Formal Report of Student Grievance
- Formal Referral for Behavioral Intervention Team (BIT)
- College Police Report
- Report of Concerning or Threatening Behavior
- Title IX Report (Gender Based Discrimination, Sexual Violence, Sexual Harassment, Cyber Bullying or Stalking)
- Referral for Basic Needs Assistance/Resources
- Informal Complaints or General Concerns
The Office of Student Services (OSS), Campus Police, or Title IX Office will respond. Please know that you and the witnesses you identify in the report might be contacted by one of the responding departments for further information about the incident.
FACULTY/STAFF - If you are reporting an incident of abuse that was disclosed to you, please use this Incident Report form.
STUDENTS/FACULTY/STAFF- If you would like to report a Title IX Complaint (matters related to Gender Based Discrimination, Sexual Violence, Sexual Harassment, Cyber Bullying or Stalking), please use this form and select "Title IX" as the nature of the report. If you have questions on whether an incident should be reported to Title IX, please contact the Title IX Office at swcertix@swccd.edu or (619) 482-6329. If you would like to speak with a confidential Liaison regarding any of these topics please call the Sexual Assault Crisis Center at (888) 385-4657.
Please note that disciplinary records are protected by federal privacy law called the Family Educational Rights and Privacy Act (FERPA).

Reports of Student Misconduct and Academic Dishonesty: If you are submitting a Report of Student Misconduct or Academic Dishonesty, before completing this form, please be sure you are referencing the Standards of Student Conduct outlined in Administrative Procedure 5500 and following the process outlined in SWC Administrative Procedure 5520, which includes the staff or faculty member first attempting to resolve the misconduct by informal consultation with the student and then the School/Center Dean or Administrator meeting with the student in an attempt to resolve the problem. Please note that informal consultation with the student need not to occur in cases where the College Police has become involved, or in situations that involve safety.
