Every effort is made by the University to preserve the privacy of reports. Although our office is not a confidential resource, we will not automatically share the report with the accused person(s) or others.

This form is designed to provide students, staff, faculty, vendors, visitors, parents/ guardians or others with an online ability, available 24/7, to report concerns related to an alleged incident(s) of discrimination based on your membership in a protected class.

Such discriminatory conduct may include: discriminatory harassment, sexual harassment (including sexual assault, dating violence, domestic violence, stalking and sexual exploitation), and/or retaliation.

A report may be accompanied by a request for resources and support only, or to request a meeting with a member of the Office of Equal Opportunity & Title IX team. Our team will make every effort to respond within 1-2 business days.
