This form should ONLY be used AFTER you have completed an "Academic Integrity Reporting Form" and have received clearance to handle the matter as a first-time Level 1 or 2 matter. If you have not completed this initial reporting form, please do so here: Academic Integrity Reporting Form. This form MUST be completed within ten days of your meeting with the student. If the student did not respond or chose not to meet with you, you may make a decision regarding the matter and complete this form after 10 working days from your original formal written request to meet with the student.
