The HRSA Maternal and Child Health Bureau has funded this Collaborative Improvement and Innovation Network (CoIIN) to test and spread promising care delivery strategies and payment models for Children with Medical Complexity (CMC). The three-fold goals of the project are to:

Improve the quality of life for children with medically complex conditions

Improve the well-being of their families

3

Increase the cost-effectiveness of their care

Project implementation took place from August 1st, 2017 to July 31st, 2021. The project is now in a supplemental fifth year focused on sustainability, spread, dissemination, and evaluation.

NEW: The CoIIN now has a materials library of products developed during the course of project implementation. This library includes both network-level and state-specific materials. CMC CoIIN state team members can submit products and resources for the CMC CoIIN website through this form.
