Individual Health Plans (IHPs) allow schools to plan for and respond to students who have life-threatening or severely impacting health conditions. Please communicate with your School Nurse to let them know about any health concerns that would impact your child’s learning or safety at school.  With input from you, your child’s Health Care Provider, and the School Nurse’s expertise in school health services, an Individual Health Plan can be developed to help your child be safe, healthy and successful at school.
