The Health Services within Geary County Schools USD 475 is made up of school nurses within school buildings who strive to provide qualify and professional care for students. These professionals meet State and Federal standards and work to maintain quality health operations on a day-to-day basis.

Physical Exam Requirements

Students eight years old and younger

Every student age eight or under entering a Kansas public or private school for the first time must turn in a physical exam form to the school. If a physical exam has not been completed at the time of enrollment, a “Physical Exam Exemption Form” must be signed and turned in to the school. The Exemption Form gives the student 90 days from the date of the enrollment to have the exam completed and the form returned to school.

A physical exam completed one year prior to enrollment in a Kansas school may be used to fulfill this requirement. Please check with the school nurse to see if a previous exam fulfills the Kansas requirement.

Forms from other schools/states are accepted.

Students in grades 6-12 wishing to participate in Kansas State High School Activities Association sponsored activities/athletic programs.

Physical exams are required for participation in Middle School and High School Athletics/Activities sponsored by the Kansas High School Activities Association.

Students must pass a physical exam with documentation on the Kansas State High School Activities Pre-Participation Physical Evaluation form. The physical exam must be completed after May 1 of the academic year for which participation is desired. For more information contact the school’s athletic director or principal.
